Virtual Disc Golf Scorecard Summary Report Form

This form allows Intramural Virtual Disc Golf participants to submit the results of their disc golf round.

Once you have entered all the information on this form, please upload a photo of your filled-in scorecard.

After submitting this form, your results will be recorded on the leaderboard for your division.  At the event deadline (May 1), the participant in the Front Nine Division and the participant in the Back Nine Division with the lowest score will be declared the winner in each league.
